Family Plan
$1100 Annual Fee for a family of four
- 2 dental cleanings
- 2 Doctor exams during dental cleaning visits

- Fluoride treatments, when indicated
- All Necessary x-rays
- Unlimited emergency care exams and x-rays
- $300 off ClearCorrect treatment
- $100 of full mouth whitening

Perio Plan
$685 Annual Fee for patients 13 and older
- Up to 4 dental cleanings
- 2 Doctor exams during dental cleaning visits
- 2 Oral Cancer screenings
- Fluoride treatments, when indicated
- All Necessary x-rays
- Unlimited emergency care exams and x-rays
- $300 off ClearCorrect treatment
- $100 of full mouth whitening
- 16% off all other treatments
Child Plan
$250 Annual Fee for patients 13 and under

Plan Details & Limitations
No deductibles, No pre-authorizations, No claim forms, No waiting period, No exclusions for elective care, Membership is valid for 12 months and nontransferable, Services cannot be filed to dental insurance, The plan is paid annually, Member discount is not valid with other promotions, No refunds.
